Endodontic treatment

Endodontics is a branch of dentistry that focuses on the treatment of the tooth’s root canals. Any medical intervention involving the root canals is performed by an endodontic dentist (endodontist).

The tooth roots anchor the tooth in the jaw (alveolar) bone. Alveolar neurovascular bundles pass through the root canals, so a serious disease of the tooth root almost never goes unnoticed, as it is accompanied by severe pain, ranging from sharp to dull, aching pain.

What is endodontic treatment

Endodontic treatment is aimed at thoroughly cleaning the tooth canals from inflammation, infection, and granulation tissue. The most common reason for endodontic treatment is untreated tooth decay that has affected the dental pulp and nerve endings (pulpitis). In other common cases, decay has spread beyond the tooth root, resulting in a condition known as periodontitis. An endodontic dentist cleans, disinfects, shapes and, at the final stage of treatment, seals the root canals.

There are two methods of treating root canals: therapeutic and surgical.

The therapeutic method is used only when pulpitis is diagnosed. Its key feature is that the nerve endings are treated rather than removed, so after treatment the tooth remains a living organ.

The surgical method is used in selected cases of pulpitis and in all other diagnoses. The pulp and tooth nerve are completely removed, after which the tooth is no longer vital, but it continues to perform its main functions – chewing and aesthetics.

When is endodontic treatment needed

Pulpitis
Inflammation of the soft tissues of the tooth (pulp) caused by the rapid progression of dental caries. Symptoms of pulpitis: sharp, throbbing pain; acute pain in response to hot/cold; radiating pain from the tooth to the neck area, more often at night.

Periodontitis
An inflammatory process of the hard tissues (jaw bone) in the area of the apex (tip) of the tooth root. Symptoms of periodontitis: dull aching pain, sharp pain, a feeling of pressure or fullness, a sensation as if the tooth has become longer, pain when biting, bad breath, purulent discharge, swelling.

Pulp trauma
Destruction of the soft tissues of the tooth as a result of trauma to the coronal part, that is, its chipping or fracture. The extent of the injury is assessed on an X-ray. Symptoms: in the first hours it is usually asymptomatic. If you do not seek help in time, sharp or dull pain, bad breath, and a strong reaction to hot or cold may occur.

Secondary endodontic treatment
Secondary endodontic treatment, or retreatment of previously treated root canals. Poorly sealed canals, extrusion of filling material beyond the root apex or root perforation, a fractured instrument left in the root canals, or a planned replacement of prosthetic restorations – all of these are indications for secondary endodontic treatment

Preparation for prosthetic treatment
In most, though not all, clinical cases, root canal treatment is performed before placing prosthetic restorations. This procedure is carried out to prevent inflammatory processes from developing under the crown or bridge.

STEP 1

Anaesthesia

Root canal treatment is usually performed under local anaesthesia, and in some cases, at the patient’s request, stronger methods of anaesthesia can be used.
STEP 2

Isolation

The dentist isolates the tooth from external factors using a rubber dam system.
STEP 3

Removal of carious lesions

The dentist removes carious lesions using the conventional method with a dental bur. This provides access to the tooth pulp.
STEP 4

Removal of the tooth nerve

Using an endodontic handpiece, the dentist completely removes the pulp. This instrument allows the nerve to be removed in a single step and in full. At this stage, and up to the moment of filling, the dentist uses a dental microscope to perform the work as precisely as possible.
STEP 5

Determining canal length

The endodontist uses endodontic files to determine the working length of the root canals. This helps prevent extending beyond the apex of the tooth root and is necessary for high-quality, hermetic filling. At the same time, the measurements are controlled using an apex locator.
STEP 6

Mechanical canal preparation

Because root canals are very narrow and irregular in shape, they need to be “prepared”, that is, mechanically widened for proper filling. For this, an endodontic handpiece with files is used.
STEP 7

Placement of medication

First, the dentist performs antiseptic treatment of the canals. After that, medication is placed inside for 3–10 days to relieve the inflammatory process.
STEP 8

Temporary filling

To prevent infection and pathogenic bacteria from entering the open canals, and to keep the medication in place inside the canal, the tooth is sealed with a temporary filling.
STEP 9

Irrigation

At the next visit, the dentist will remove the temporary filling, irrigate the canals and perform antiseptic treatment of the tooth.
STEP 10

Drying

The canals are dried using special paper points. This is done to ensure good adhesion of the filling material to the inner walls of the tooth root.
STEP 11

Permanent filling

For root canal filling, our clinic primarily uses warm gutta-percha. Under the influence of temperature, the material becomes flowable and fills all spaces, even microscopic ones. At the same time, no mechanical pressure is exerted on the roots, which prevents their fracture during filling and in the future (for example, due to microcracks).
STEP 12

Control X-ray

The dentist must perform an X-ray examination of the filled tooth, verify the accuracy of the work, show the image to the patient, and add it to the patient’s medical record.
STEP 13

Tooth restoration

Once we have confirmed the quality of the treated root canals, we can proceed to restoring the tooth crown or to its prosthetic restoration.

Frequently asked questions about root canal treatment

How many visits does endodontic treatment take?

Depending on the clinical case, the number of visits to the endodontist can range from 1 to 4. In some diagnoses, for example in acute periodontitis, the dentist places medication in the canals for several days and then replaces it. This may be repeated several times until the desired result is achieved and the inflammation is resolved. There are also cases when all endodontic treatment can be completed in a single visit.

A tooth without a nerve has turned dark. Can this be fixed?

An examination by the dentist is necessary to determine the cause of dentine darkening. Patients who had endodontic treatment a long time ago often come with this concern, as medications widely used at that time can cause such an effect. The defect is usually corrected using internal (endodontic) tooth whitening or by placing dental veneers.

Tooth pain after nerve removal

Pain after endodontic treatment can be either a normal or a pathological condition. Moderate, tolerable, even pain during the first 2–3 days after treatment is considered normal. Residual sensations may persist for another 2–3 weeks. If the pain is sharp, increasing in the first days, does not subside, or remains the same 3 weeks after treatment, you should consult a specialist with these symptoms.

How painful is root canal treatment?

Since the procedure involves exposed nerve endings and sensitive tissues, all treatment is carried out under local anaesthesia. During treatment, the dentist can administer additional anaesthetic when its effect begins to wear off. Therefore, the patient does not experience sharp, severe pain. For those who are very afraid of dental treatment, treatment under sedation can be used additionally.

Broken instrument found on the X-ray

If the canals are well filled and the tooth does not bother you, a broken instrument does not pose a threat. If the root requires retreatment, you should find an experienced specialist who can remove the broken instrument and who works with a dental microscope. The dentist’s expertise and appropriate equipment are the key to successful treatment and avoiding damage to the tooth root.

How long will a tooth without a nerve last?

A devitalised (non-vital) tooth can last a lifetime. Its longevity depends on the quality of the treatment performed and the individual general health of the patient. That is why it is important to treat teeth and to do it properly.
