Implantation using the All-on-6 protocol
All-on-6, or “all on six,” is a one-stage dental implant protocol in which a fixed dental prosthesis is placed on 6 implants. Unlike the All-on-4 technique, all implants are placed vertically, if the anatomical features of the jaws allow it.
On the internet, All-on-6 is often described as an improved version of the All-on-4 method. However, this is not the case: implantation on six implants appeared earlier than implantation on four. As with the “all on four” method, a full fixed prosthesis is placed immediately on the day of implantation. The material used to make the dental prosthesis may vary: acrylic, metal-ceramic, or zirconia.
Methods of prosthetic rehabilitation using the All-on-6 protocol
In the technique for restoring all teeth in a jaw on six implants, additional supports are placed in the molar area. This allows the load to be distributed evenly across the entire structure, thereby extending its service life. In addition, the structure can be stabilized and reinforced with a metal framework.
With the one-stage All-on-6 implantation technique, several surgical methods can be used. The most modern and precise method is guided surgery using a surgical guide and computer modeling.
The guide acts as a directing structure for correct and precise implant positioning and helps avoid critical anatomical structures (for example, the mandibular nerve or maxillary sinuses). A navigation guide also has a drawback — the overall cost of the operation immediately increases.
Another method is immediate implant placement. When the patient still has natural teeth that need to be removed, the operation is carried out immediately: the dental implant is placed into the socket formed after extraction. If teeth that are not located in the planned implant placement area need to be removed, they are also extracted on the day of implantation.
Another method is the traditional, classic technique with gum flap reflection. The surgeon accesses the bone for implantation by making an incision in the gum.

When agreeing to any surgical intervention, it is important to understand that it carries certain risks. During All-on-6 dental implantation, complications such as trauma or damage to bone tissue, injury to the mandibular nerve, or an unstable implant/implants may occur. Treatment time for implantation using the All-on-6 protocol
All-on-6 surgery takes about three hours. A temporary prosthesis can be placed immediately after surgery or with a delay (no later than the third day). Osseointegration of the implants in the bone takes approximately 6 months. Once complete osseointegration has been achieved, a permanent prosthesis can be placed.
In some clinical cases, when indicated, a permanent dental prosthesis is placed immediately. In this case, the total implantation and prosthetic treatment time will be 1 day, provided that the clinic, in addition to a surgical office, has the necessary technical facilities to fabricate a surgical guide and dental prosthesis.
