What is a sinus lift?
A sinus lift is a bone grafting procedure (osteoplasty) used to build up insufficient bone tissue in the upper jaw in the sinus area (maxillary sinuses).
This procedure is performed before dental implantation when there is a deficiency of bone tissue in the area where the future implant will be placed.
Types of sinus lift
Sinus lift procedures are divided into closed (vertical) and open (lateral). The main criterion for choosing one method or the other is the degree of bone tissue atrophy. The goal of both sinus lift methods is to elevate the “floor” of the maxillary sinus and fill the void formed in the alveolar bone with bone material.
To perform a closed sinus lift, the bone height must be at least 5 mm; accordingly, in all other cases an open sinus lift is performed.
The surgical technique differs significantly. In a closed sinus lift, access to the jawbone is made through a small opening in the bone, without incising the soft tissues, while in an open sinus lift access is obtained by reflecting a gum flap. An important feature: if there is 5 mm or more of bone tissue, an open sinus lift can be performed and an implant can be placed immediately. A disadvantage of this approach is the inability to apply immediate loading (prosthetic restoration).
With a closed sinus lift, we can also place an implant in one stage, but this method is used when only one tooth is missing. If several teeth are missing in a row, even when 5 mm of bone is present, the open sinus technique is used.
Open sinus lift
Advantages:
- allows restoration of a significant bone deficiency
- provides sufficient access for the surgeon
Disadvantages:
- implant placement is delayed for 3-6 months
- relatively traumatic method
- difficult recovery period; prosthetic restoration on the implant is possible after 3-6 months
- requires special skills from the doctor
- requires suturing of the wound
Closed sinus lift
Advantages:
- implant placement can be performed immediately after the sinus lift
- minimally traumatic method
- does not require suturing of the wound
- easy recovery period
- one-stage prosthetic restoration is possible
Disadvantages:
- cannot be used in cases of significant bone deficiency
- due to limited visibility, there is a risk of damaging the Schneiderian membrane (also called the Schneider membrane or Schneider's membrane)
Sinus lift: is it worth it?
Sinus lift allows restoration of bone volume in the area of a missing tooth. This, in turn, makes it possible to place a dental implant. Oral surgeon and implantologist Maksym Makarenko explains the nuances of the procedure.
Bone graft materials for sinus lift
Autograft
The patient’s own biomaterial from another part of the body serves as the bone graft. Most often, this is bone from the lower jaw.
Allograft
The graft is a bone fragment from the same biological species, in this case from a human donor.
Alloplastic graft
An inert, artificial bone substitute that is placed into the human body (collagen membrane, bioactive glass, or calcium-substituted silicon oxide)
Xenograft
In this case, bone material of animal origin is used after prior purification and sterilization. Most often, the donors are cattle or domestic animals.

Recommendations before and after a sinus lift
For the procedure to go successfully and without complications, the stage of preparing the patient for surgery is very important. We recommend:
Before the procedure:
- Get enough rest the day before surgery
- Limit alcohol consumption the day before surgery
- Eat 1-2 hours before surgery
- Brush your teeth thoroughly before your visit to the clinic
After the procedure:
- Replace your toothbrush with a new one with soft bristles
- Avoid pressure changes in the head area for 2-3 weeks: do not dive, do not puff out your cheeks, do not rinse your mouth, do not play wind instruments, avoid positions with your head down, etc.
- For 10 days after the procedure, avoid overheating and overcooling the body
- Refrain from harmful habits during the period of medication therapy
- Do not eat for two hours after the procedure. Thereafter, eat soft, warm food and chew on the side opposite the surgical site.
Complications after sinus lift
Swelling
Swelling is present for 3-5 days after a sinus lift. This is a normal reaction of the body to trauma. The peak occurs on the third day, after which the swelling gradually subsides. If the swelling continues to increase on the fourth day, contact your doctor immediately.
Increased body temperature
An elevated temperature during the first two days, up to 38 degrees, is considered normal. There is no need to lower such a temperature. A body temperature above 38 degrees Celsius and a prolonged elevation in temperature (on the third day and later) are signs that specialist help is needed; seek medical advice.
Pain
Pain caused by the trauma may accompany you during the first 2-3 days and usually occurs in the second half of the day. Aching or pulling pain around the surgical site is normal, and the symptoms can be relieved with medication after prior agreement with your doctor. Sharp, increasing pain, throbbing pain, pain that does not subside, or pain localized outside the surgical wound are reasons to contact a specialist.
Exacerbation of chronic sinusitis
Surgical intervention causes swelling, and in patients with sinusitis it is already present, so the swelling increases, presses on the Schneiderian membrane, and if the membrane cannot withstand it, it ruptures and purulent contents enter the maxillary sinus. In such a case, surgical intervention will be needed to clean the maxillary sinus. Accurate diagnostics and the doctor’s skills help reduce the risk of such a complication.
What alternatives to sinus lift are available
Sinus lift is performed only before dental implant surgery, so let’s look at the options for implantation and prosthetics without using a sinus lift.
Short implants
If there is a minimum amount of bone tissue, dental implants can be placed using short implants. For this, the bone height at the site of the future implant must be at least 4 mm. However, if the implant is rejected by the body, the resulting socket will be wider than the standard implant size.
Further prosthetic treatment will then require additional time and money to restore the bone tissue. Another difficulty is finding a surgeon, because short implants require special skills: while a conventional implant is screwed in, a short implant is impacted into the bone, and not all surgeons have this skill.
Implant placement bypassing the sinus
Perhaps one of the best ways to avoid a sinus lift in terms of reliability, durability, and aesthetics.
As with short implants, the main condition for using this technique is sufficient existing bone volume (at least 6 mm).
If these conditions are met and the patient has no other contraindications, an implant can be placed bypassing the sinus (usually in the area of the maxillary tuberosity), and at the prosthetic stage an aesthetic ceramic restoration can be fixed.
Basal implantation
Basal implants are placed not in the alveolar bone but in the basal bone, which is located deeper. The implants themselves are quite large, and therefore, if the human body rejects this foreign body, a rather large defect will remain in the bone and will have to be restored surgically. In addition, basal implantation is a fairly traumatic method.
Doctors use this method when all other ways of restoring the tooth are impossible.
Dental bridge
If 1-2 teeth are missing, a dental bridge supported by adjacent teeth can be placed. The advantages of this method are a short treatment time, relatively affordable cost, a high aesthetic and functional result, and the non-invasive nature of the technique.
There are also disadvantages: over time (after 7-15 years, depending on the patient’s age and individual characteristics), the bone will continue to atrophy and a gap will appear between the gum line and the bridge. In this case, the restoration will need to be replaced and the gum line corrected, or you will need to choose implantation or a removable denture.
Removable prosthetics
The functionality of the tooth can be restored with dentures. Their use should also be based on indications. Because removable dentures do not require surgery, the time needed to restore the dentition is much shorter (up to 14 days), and the cost is lower. But there are also drawbacks: dentures require regular replacement (every 3-5 years), may break, and also contribute to bone atrophy, which over time will only worsen the overall condition of the oral cavity.

In such a case, it is better to listen to the surgeon, because they take all risks into account, including the risk of damaging the membrane due to limited visibility during a closed sinus lift.
How painful is a sinus lift?
The procedure is performed under anesthesia using modern anesthetics, so painful sensations are completely absent.
Can I travel by plane after a sinus lift?
Yes, you can, approximately one month after the procedure. After a sinus lift, changes in atmospheric pressure should be avoided; air travel is prohibited for the period determined by the doctor, usually at least one month.
Is it possible for the bone graft not to integrate?
Yes, there is a risk that the bone material will not integrate into the recipient’s bone tissue. A healthy lifestyle and following the doctor’s recommendations will help reduce this probability.
Does healing take a long time?
Healing of the gums takes about two weeks. Full osseointegration takes from 3 to 12 months, depending on the type of procedure, the initial conditions (available bone volume), and the individual characteristics of the patient’s body.
What should I do if the bone graft did not integrate?
A repeat procedure can be performed no earlier than after 6 months, with replacement of the bone material and intensified medication support during the rehabilitation period. The second option is to consider alternative ways of restoring the dentition with your doctor, without using a sinus lift.
